The Questionnaire is a special container that can hold either Choose One, Score by Category, or Select Multiple tools. The container displays the content of these tools in tabular format, as illustrated below. The "questions" appear in the first column, and the "answers" appear in subsequent columns. You also have the option of including a "Comment" column, in which the mobile user can enter any comments they wish.

Note the following:

You may not mix question types (Choose One, Score by Category, and Select Multiple) within the same questionnaire.

The "answer" options must be exactly the same for all of the "questions." This includes the answer name as presented to the user and underlying value properties for the individual tools.
Properties
Questionnaire Label: This label appears at the top of the questionnaire in black.
Fit to screen width: This feature provides a level of device responsiveness as it attempts to force your container elements to fit within the available width of your device's screen. If Fit to screen width is not selected, then the container is allowed to stretch beyond the right border of your device's screen, which may be preferable if your grid contains a large number of elements.
- Equally space columns: Attempts to divide the screen width of your device by the number of columns in your container.
- Use caption length: This setting will attempt to use the width of each of the tool's Answers to set the width of the columns.

Adjust column width: (The Fit to screen checkbox must be selected in order to use this option.) This option gives you the most control over how the questionnaire's columns widths are set. When you select this option, column setting fields will appear in which you must enter values that equal 100%. For example, if you had a questionnaire with Question, Answer, and Comment columns, you might to allocate 30% of the screen to the Question column, 45% of the screen to the Answers, and 25% of the screen to the Comments. The example below shows how you would enter those settings:
However, note that the width allocated to the Answers will be divided equally between all of the possible answers. So, if your questionnaire has three available answers, then the 45% of the screen (that we already allocated, above) will be divided equally between the three possible answers (i.e., 15% apiece).
Append comment field: This option allows form users to add a comment for each question/answer (e.g., a comment about why they thought a product was "Bad"). If you leave the Comment label field blank, the label for the Comment column will be "Comment." You can change this by entering your own label in the Comment label field (e.g., "More Info").
