1. Use the standard Windows uninstall
2. Delete the folder C:\ProgramData\doForms\Sync And Save
3. Delete the folder C:\Users\[User Name]\AppData\Roaming\doForms\Sync And Save
4. Delete the folder C:\Users\[User Name]\AppData\Local\doForms\SyncAndSave.exe...

For steps 3 and 4 you will need to turn on the "Show hidden files, folders, and drives" option in Windows:
